What do Social Prescriber’s do?
In the UK, one in five GP appointments is related to wider social needs rather than medical issues.
Social prescribers (also called social prescribing link workers) work together with GP practices to ensure that the wider needs of patients are addressed.
Our Social Prescriber’s role is to connect patients to groups, activities and services in their communities to meet their social, emotional and practical needs that affect their health and wellbeing. In that role, our Social Prescriber can be the bridge between our GP’s and all the non-healthcare services available in Stokenchurch.
Social prescribing can help you if a patient is:
- Struggling with finances and need some advice
- Feeling low, stressed or down
- Feeling lonely or isolated
- Want to improve your management of a new or long-term health condition
- Need help getting housing or employment advice
- Want to get more active
- Experiencing a change, for example a job loss, relationship breakdown or bereavement
- Needing to talk through multiple things affecting your wellbeing
Link workers also help people with complex social needs.
Generally, patients speak with a link worker 6-12 times over a three month period. The link worker will connect them with activities, groups, and services in their community. This is for practical and emotional support to help them get back on their feet.
